To replicate, add a flac album with no artwork embedded in the files and no artwork jpeg file. Rescan. Verify that no artwork was found for the album. Now add cover.jpg to the directory and re-scan, artwork is ignored and the album still has no artwork.

Matt, to clarify when you say rescan do you mean "Clear library and rescan everything" or "Look for new and changed music"?

This has always been a problem with "Look for new and changed music" scans.

See bug 2931 and a host of others.  This is an old, old  issue.  Ultimately, if you can follow all the duplicate bug links, you'll see that it was tossed into the growing pile of bugs that are going to magically fix themselves with the new database schema.
