patch which adds:

1) Support for NAD SR5 remote.

2) Supports arbitrary repeatcode for remotes (most use 00000000).

3) Adds support for tri-state select of which remotes are enabled (user-enabled, user-disabled, 
slimserver-default).

I'm not too familiar with the IR bits.. will try to have a look.

high likelyhood this will clash with bug 527.  one or the other will need
rework, which I'll investigate.  can't really test this patch too much since I
have no such remotes which make use of the features added here.
