It only shows up in one place, under Now Playing.  And only for the currently playing track...  This is 
with the default skin.

can you explain this in more detail?  It seems this problem is either gone, or
I'm not following exactly where and under what conditions this occurs.

i think it's still showing this.  if you set the title format in server settings
-> formatting, it's supposed to change the display on the right, i'd think...

but if you set it to just TITLE it still shows TITLE from ARTIST on the right
hand side under the current playlist.

most of the listings, at least in fishbone and default skins will display TITLE
by ARTIST from ALBUM, if TITLE doesn't already include artist or album.  This
allows the line to include links to the related album and artist.  if the
formatting is set to TITLE - ARTIST - (TRACKNUM) ALBUM, then you get a matching
formatted output, but it links ONLY to the track, or in the case of the current
playlist it will link to playing that track.

If you are referring to the status_header, I have less input on that. I can't
recall how the default skin is set up on that.

Uh, I feel stupid here, I can't remember specifically where I was setting this,
and it doesn't seem to be an issue any more anyhow.  Sorry...:(
