patch to deliver customize home menu option as item context menu

This is a patch to deliver customize home menu in context menus for any
SlimMenu item (My Music top menus, Settings, Advanced Settings, etc.).

What it doesn't do (and why I cc:ed you Michael), is give context menus for the
items in My Apps. This is a regression from what's available in the current
Settings->Home Menu, because we send special hidden items for each of those via
Slim::Plugin::OPMLBased.pm.

I've been thinking about this for a while and can't figure out a simple way of
keeping this function with the change that's proposed here. 

In other words, what I'm looking to do is make a context menu for adding to
home menu work for items that are listed under My Apps
