With both USB and SD card inserted if you eject a device you get an eject failure if it was the second device inserted. If you first eject the first device that was inserted the second will eject okay. 

Insert USB, Insert SD 
eject SD  receive eject error

insert SD, insert USB
eject USB  receive eject error

insert SD, insert USB
eject SD  ejects okay
eject USB ejects okay

insert USB, insert SDB
eject USB  ejects okay
eject SD ejects okay

Working fine here, any combination.

- what error message do you get?
- what firmware revision are you running?
- anything more interesting in /var/log/messages?

the message
"warning the device failed to eject properly. This may be due to one or more programs on the system accessing it."

it may just be a timing issue as today I selected retry and it then ejected okay
