Allow multiple streams from the same IP address by specifying player=id_string in query string

Attached is a patch which allows multiple streaming clients from the same IP address by specifying arbitrary player IDs using a URL of http://server:9000/stream.mp3?player=id_string. It would be useful to be able to have multiple different streams originating from the same IP address to allow Squeezebox Server's streaming to be used behind some form of Port Forward/SSH Tunnel, or from a multi-user system. This would allow someone to SSH Tunnel a remote VLC player, without Squeezebox Server getting it confused with the SqueezePlay client running on the same host as the SSH server.
