It appears the headphone detect doesn't switch over to headphones if the headphones are plugged in at boot time.

I had this last night:
* Battery installed
* Shutdown.
* Power up, with headphones installed
* Play something

Sound came out the speakers.  Unplugging and re-plugging the headphones fixed it.

Caleb: what hardware/firmware versions? I can't repo this on a PVT (PA5) with r7707 or later

testing with r7790, I have not been able to replicate this yet.
