on a new account, if I use on-device APP Guide to add an app to my SP device, the new app will fail to install into my apps, UNLESS I tell it to add to my home menu during install

Hmm this works for me.  Selecting either home menu or not adds the app to My 
Apps.

I am still able to reproduce this - intermittantly

Go to Test.SN
Remove all apps from My Apps
Go to your SP device connected to Test.sn
Select My Apps and verify you have no apps (should get warning message)
On Device, go to App Gallery
Select Music On-Demand
Select Napster
Select Install Napster
Select Don't add shortcut to home menu
Select Launch Napster Now
Click HOME button
Select My Apps
Notice Napster is not listed

James - are you saying you think this was fixed? AFAICS you were still able to intermittently reproduce and nobody says they fixed it.

Works for me for now.  Still a bit worried about this but it's not that severe, you can wait a short time and then My Apps will work apparently.
