I've only seen this with favorites, but it may happen in other menus.  I've also only seen it using the IR interface.

1) Navigate to Favorites and scroll down to something not on the initial screen
2) Play it
3) Return Home using the IR Home button
4) Navigate to Favorites
5) Notice that you are at the top of the favorites menu
6) Press down-arrow
7) Notice that you have jumped to the entry below the fav you are playing
